We accept most major insurance plans. Please call our office (208-535-4567) to inquire whether we participate with your particular insurance plan.
Prior to your appointment, please check with your insurance company regarding referrals, co-payments, and any deductible required at time of service.
Billing
Co-payments or co-insurance are due at time of service. For your convenience, we accept cash, checks, Visa, MasterCard, American Express, and Discover.
Please notify our office of any change in name, address, phone number, or insurance information.
